THIS DOCUMENT HIGHLIGHTS THE TERMS AND CONDITIONS FOR HOME DELIVERY OF PHARMACY SERVICES AND GOODS FROM SHALOM LIFECARE MEDICAL STORE.
Customer places order with Pharmacist via in person, Telephone, WhatsApp or Email. Scanned and valid Doctor’s prescription is essential.
It is the responsibility of the customer to give the correct and valid prescription to the Pharmacist.
Please be aware of membership benefits before you purchase.
Payment modes are via online payment gateway, Google pay/Paytm, card, or cash on delivery.
Free delivery if address is within delivery limits [As of today Dahisar and Borivali East and West, and Kandivali East] and order quantity of at least Rs 500 for upto 2 Km distance, 1000 Rs for > 2 Km. If not, delivery charges applicable 100 Rs.
Delivery and order time: Orders taken from 7 PM of previous day to 6 PM of same day will be delivered after 6 PM on same working day subject to availability.
Delivery will be free if made at routine round time 6 PM onwards. If delivery is needed on urgent basis or before 6 PM, 100 Rs will be chargeable.
If patient refuses to accept delivery, a fine of Rs 100 will be charged
For payments on Delivery, Once Google or Online payment is confirmed by Pharmacists [Google pay from their mobile], the driver will hand over the product to the customer.
Name and contact number of the person taking delivery must be noted by the delivery team member on the duplicate Sales Invoice with time
